I have been advised that the wear of airbrush tanning depends on my skin's age, texture and absorbency ability and the after care that it is given. By agreeing to these terms, I therefore release CUSTOM TANNING should there be any immediate fading or loss of the airbrush tan.

I affirm I have been advised of the airbrush procedure and that the products being used are FDA approved and give my permission to processed with the airbrush application.

I also release CUSTOM TANNING and its associates from any responsibility in the event that I may not like the procedure. I will discuss the situation if I am not satisfied with the CUSTOM TANNING management in order to make arrangements that are satisfactory for both parties involved.

I affirm that I am not allergic to any dyes, inks, nuts or cosmetics to my knowledge.

I have read the Preparation and FAQs page on www.customtanningvegas.com and agree to do the following things prior to my appointment:

1. Exfoliate my body with either exfoliation wipes or salt scrub.
2. Shave my legs and underarms before my session (Women only)
3. Refrain from wearing lotion at least four hours prior to the session.
4. Wipe off any excess body oils, including make up as well as deodorant prior to my session.

I also agree to advise my tanning technician of any skin problems I may have prior to the appointment.
I have read and understood and agree to abide by the dress code set forth during the session and immediately after the session in the preparation and FAQs page of the website.
I have read and acknowledge all of the post-care guidelines outlined on www.customtanningvegas.com, which include, but are not limited to:
1. Waiting at least six to eight hours following the airbrush session to shower or get wet. It is best if I wait until the next day to shower.
2. Refrain from using lotion until after showering.
3. Avoid using products, including lotion, that contain mineral oil or of any other exfoliating ingredients.
4. Avoid using the jacuzzi until the end of the seven days.
5. Avoid using self-tanners from the drug store in lieu of proven salon tan extenders.

I am at least 16 years old and in good health, and am able to stand and bend for at least 15 minutes.

Appointment Booking Terms and Conditions
When booking an appointment within 24 hours of my appointment time, I understand that I cannot change that appointment time and should I not show up for the appointment, my bankcard will be charged.

I understand that I can only cancel an appointment if it is 24 hours or more before the appointment. If I cancel any later than that time period, my bankcard will be charged the full amount for the appointment..

Monthly Tanning Passport Terms and Conditions
Should I opt to purchase the monthly passport for airbrush tanning, I understand that my appointment will be on the same day at the same time each week during our Monthly Passport Schedule time in order to receive the discount. If there is an incident which I cannot make it at that time, I will reschedule for a later time that is within the Monthly Passport Schedule within that week. If I cannot do so I will make an appointment during regularly, designated open times and be subject to a $15 surcharge for a change in time. If for some reason my technician needs to cancel the appointment, she will give me a 24 hour notice, and make arrangements to airbrush me at the next available time. She will also compensate me $10 toward my next purchase for the inconvenience.
